What do you need to arrange?

Start with your situation, not the official form name. Each case combines practical Infopack guidance with what can be confirmed in the current Regulations.

Forms and available application types in USOS may change during the year. Check the application name and Dean’s Office notices before submitting.

§ 36-38 · Infopack · p. 13

confirmed in the Regulations

10 / 16

A review of fairness, not a third attempt

You have 2 working days from publication of the disputed grade

The application must identify detailed concerns about impartiality, consistency with requirements or the conduct of the assessment. A negative committee grade rules out conditional continuation.

Prepare before submitting

  • Date the grade was announced
  • A specific objection
  • Requirements stated in the first class
  • Evidence or a proposed observer

Application path

  1. Record the publication date.
  2. Collect materials supporting the objection.
  3. Submit detailed grounds within 2 working days.
  4. Await information about the committee and assessment form.

Draft wording to complete

I request permission for a committee [assessment/exam] in [course]. The disputed grade was announced on [date]. My objection concerns [impartiality / requirements / conduct]. Grounds: [specific facts].

Every tip has a confidence level

We do not merge rules, practical advice and non-public documents into one category. Each case shows what the Regulations confirm and what comes from the Infopack.
